Monmouth County Health Department Offers Clinical and Preventive Health Services

The Monmouth County Health Department offers a wide range of vital health services to county residents of all ages, including well visits and immunizations for children getting ready to head to camp or school. For details on these and other health services, visit

Eye Screening Clinic

In partnership with the New Jersey Commission for the Blind and Visually Impaired, the Monmouth County Health Department offers a free eye screening clinic for ages three and up the second Monday of each month from 3:00 to 6:00 p.m. at the Health Department. The clinic is for eye health, visual acuity, and glaucoma. Other health services are also provided during the clinic. For additional questions or appointments, call (732) 431-7456.

Healthy Traveler Program

The Health Department provides an International Traveler Program tailored to education, as well as vaccinations for the traveler. Individuals or families requiring the above services are welcome to call for information or to schedule an appointment. Immunization Services for Children (18 and Under)

Childhood immunizations are provided free to children from birth to 18 years old who have no insurance or are uninsured and living in Monmouth County Health Department Member Towns. With parental permission, the child/children will be entered into a statewide immunization registry, which encourages timely and age appropriate immunizations.

IMPORTANT NOTE: Effective December 1, 2016, the clinic will be run by appointment only. It is open every Monday, except holidays, from 9:00 to 11:30 a.m. and 1:00 to 3:30 p.m. The appointment can be made by calling either (732) 294-5458 or (732) 431-7456, ext. 8516. The clinic is located at 3000 Kozloski Road, Freehold. Please note these guidelines:

Patient must be accompanied by a legal parent and/or guardian; identification must be shown. If the school nurse completes his or her assessment of patient records, please attach a copy of those documents for the patient to present at clinic. If no documentation is presented at school, please make a note on your form and/or letter that accompanies the patient at clinic. Should the clinic be closed, they make every effort to update their phone message, but please understand emergencies do happen.

Well Child Physicals

Free physicals are available for children living within MCHD member towns from birth through 18 years old.  If the child does not live in one of these towns, the fee is $20. Held on the third Thursday of the month, in the morning by appointment only, at the Monmouth County Board of Social Services, 3000 Kozloski Road, Freehold.
